DFP Recruitment are working with Open Arms who are seeking a Clinical Care Coordinator to join their team on a 12 month contract with possible extensions. This role is a Full Time role (Mon - Fri) and will be based in Launceston.

The position of Clinical Care Coordinator entails working collaboratively with clients and internal and external stakeholders to enhance the ability to identify and respond to the needs of vulnerable and at risk veterans, current & former Defence Force members and/or their families.

The Clinical Care Coordinator will take a strengths-based, recover approach to supporting vulnerable and/or at risk clients to enhance wellbeing and participation in their community through community engagement, collaborative recovery work, case coordination and complex case management.

The role requires you to:

  • Liaise and form partnerships with various internal and external stakeholders
  • Direct provision of high quality, evidence informed clinical services including intake, assessment and referral, complex case management, and case coordination.
  • Maintain clinical records, collating and analysing data from client records & preparing reports as required
  • Work systemically with key internal and external stakeholders to influence a recovery approach within a stepped care model for vulnerable or at risk veteran and defence force communities and families
  • Participate in community development and service promotion activities
  • Contribute to the development and delivery of group programs
About you:
  • Qualifications must be that of a mental health professional.
  • Qualified health professionals must have current registration with the Australian Health Practitioners Registration Agency (AHPRA).
  • Mental health accredited social workers must be registered with the Australian Association of Social Workers (AASW).
  • Preference is for mental health professionals including Clinical Psychologists, Psychologists, Mental Health Occupational Therapists or Mental Health Social Workers
  • Working with Children (WWC) and/or Vulnerable People Checks/Clearance must be obtained prior to commencing in the position.
  • An Australian Citizen and willing to undertake a National Police Clearance.
