Job Description

Carer Support Worker, Perth PKC07233 £26,372 - £29,225 New Rannoch Centre, Shuna Court, North Muirton, Perth PH1 3DN An exciting opportunity has arisen for supporting adult carers of all ages, with specific focus on carers of people who have been diagnosed with a Terminal Illness. You will be part of a positive and proactive team working across all Perth and Kinross localities adopting a holistic approach to supporting the carer and cared for, through assessment, reviews and evaluation. You will be required to work with Carers to complete their Adult Carers Support Plan, ensuring that identified resources will be available and effective to assist them in their caring role. You will be responsible for developing and co-ordinating a telephone/virtual and outreach befriending scheme. Your role will also include and allow you to develop a quality assurance approach, promoting and managing best practice, whilst facilitating partnership working and community engagement within localities. You will have the opportunity to progress the above roles in a variety of community settings. You should have a genuine interest in the health and wellbeing of carers and the people they care for. The successful candidate will be enthusiastic and motivated, have a keen interest and willingness to deliver a high standard and fast moving service to support individuals through their journey. It is essential that you have excellent communication skills and the ability to build relationships and communicate at varying levels with carers, cared for, colleagues and other professionals. You should also have the ability to work within established procedures and best practice guidelines, whilst being able to plan, organise and supervise your workload in line with regulatory requirements.

The Individual
You should be confident with supporting collaborative and partnership working by engaging and working alongside other professionals, support groups and external agencies to support the carer and the cared for. You are required to be competent in a range of IT skills and quality assurance approaches. You will have proven experience in assessment processes supporting the carer and cared for individual. You will have the ability to use your initiative and make decisions, whilst also being a good team player and knowing when to seek further advice from a manager. Take responsibility for yourself, carers/ care for, contractors and others and ensure compliance with all relevant Health & Safety legislation. Promote and act as a role model for the Council's values and commitment to equality and diversity. In addition promote positive values and anti-discriminatory, non judgemental practise and treat colleagues and customers with dignity and respect in line with the SSSC Codes of Conduct. SVQ3 in Health and Social Care is required. Due to the geographical range of this job it is essential that you drive.
